Liability Waiver

NOTE: Minors under the age of 18 attending without parents, must print this form, have it signed by the parents, bring it to the event, and give it to the organizer/check-in crew. Everyone else must sign the electronic waiver HERE, otherwise you’ll be asked to do the same at the event before you are able to serve.

Project: Finding Calcutta Liability Release and Waiver Agreement

This Liability Release and Waiver Agreement (Agreement) is entered into by and between Project: Finding Calcutta (PFC), its principals, employees and volunteers and the individual (herein referred to as Volunteer or Volunteers), who by signing below, release PFC from any and all liability and release PFC from any harm or claims of damage. Volunteer accepts the terms of this Agreement and agrees to abide by the following terms and conditions.

Volunteers participating in the activity/program, offered by PFC acknowledge and agree their participation is voluntary and on their own free will. Volunteers recognize that the activity/program requires physical exertion and commuting to and from the location of the activity that may be strenuous and may cause physical injury and they are fully aware of the risks and hazards involved. Volunteers understand that it is their responsibility to consult with a physician prior to and regarding their participation in the activity/program. Volunteers represent and warrant that they are physically fit, and they have no medical condition that would prevent their full participation in the activity/program.

In consideration of being permitted to participate in the activity/program, Volunteers agree to assume full responsibility for any risks, injuries or damages, known or unknown, which might incur as a result of participating in the activity/program. In further consideration of being permitted to participate in activity/program, volunteers knowingly, voluntarily and expressly waive any claim they may have against PFC for damages, and injury, including death that they may sustain as a result of voluntary participation in the activity/program. Volunteers, their heirs or legal representatives forever release, waive, discharge and covenant not to sue PFC, Ask a Bishop (the parent organization), Catholic Diocese of the location in which they are serving, or the owners or employees of the activity/program at which Volunteer volunteers, as well as of the meeting location used for the arrival or departure of the activity/program for any injury or death caused by voluntary participation in the activity/program.

Volunteers are agreeing that during their participation in the activity/program, their likeness may be captured through photographs and/or video recording with/without sound for the purpose of preserving memories and/or for media/advertisement, including posting their likeness on the PFC website, social networks, or other media efforts. Volunteers hereby grant permission for their likeness to be used, until revoked. Volunteers understand revocation is not effective until they have provided their revocation in writing to PFC. Volunteers understand that revocation of permission will not include any likenesses of them already used for above purposes.

Volunteers agree to read the above Agreement and fully understand its contents. Volunteers freely agree to the terms and conditions stated above. This agreement remains in effect for as long as Volunteer participates in PFC activities/programs.
